Consent Form

Study Purpose

This program is being presented by the University of North Carolina School of Public Health departments of Nutrition and Maternal and Child Health, and the Maternal and Child Health Bureau Distance Learning Grant. The purpose of this study is to evaluate the promotion and reach of the Public Health Nutrition for the 21st Century online program, developed to advance the analytical, managerial, and clinical skills of public health professionals, specifically those serving women, children, and families.

What will happen during the study

This study will be completed entirely online. You may complete either one, two, three, or all six of the available modules. It should take up to 2 hours to complete one module, depending on the subject matter and your baseline knowledge and skills. The system will be programmed so that a module can be 'saved' and 'resumed' later. Because this is an evaluation project, you will need to register and consent to participate.

Each module is divided into six parts and is available in two formats: PowerPoint only or PowerPoint with audio narration. At the end of each part, you will be asked to answer five questions related to what you learned in that section. You must answer at least four of these questions correctly in order to proceed (quizzes can be re-taken after reviewing the associated section). You will also be asked to complete a survey upon completion of the module to provide feedback on the module itself. Upon completion of the entire module, including the follow-up evaluation, you will be able to print a Certificate of Completion.

Benefits to you

This research study is designed to address barriers that public health professionals like yourself encounter when interested in acquiring skills for linking nutrition research to action. The content of the modules will provide you with an introduction to research and the practice of public health nutrition, and will help with nutrition problem solving and program planning to improve the health of your clients. A Certificate of Completion will also be available for each module completed.

Risks and rights

The only cost to you is your time to participate in the study and possibly the cost of internet access. The only risk to you might be if your identity were ever revealed. You will be assigned a participant number during this study. Although you must enter your name for the Certificate of Completion, your name will not be used for the research components of this study.

Participants will not be identified in any report or publication about this study. Although every effort will be made to keep research records private, there may be times when federal or state law requires the disclosure of such records, including personal information. This is very unlikely, but if disclosure is ever required, UNC-Chapel Hill will take steps allowable by law to protect the privacy of personal information. In some cases, your information in this research study could be reviewed by representatives of the University, research sponsors, or government agencies for purposes such as quality control or safety.

Participation in this program will not affect your job or the services you are required to provide. We will publish results from this research study as we think best for research and teaching purposes. You may choose not to be in the program or to stop being in the study before it is over at any time.
